To Request a Transcript
Read the information below. Then submit your request in writing, with a $5 fee for each transcript (check or money order payable to Saint Vincent Health Center) to:

About Transcript Requests
For official transcripts, the following information must be given at the time of request:
- Name
- Name when attending the school
- Social security number
- Year of attendance and graduation
- Name and address of the educational institution, or individual, to which the transcript is being sent.
Official transcripts must contain the seal of the Saint Vincent School of Nursing.
A self-addressed postcard will be sent to the educational institution with the transcripts and maintained on file in the Saint Vincent Health Sciences Library upon its return for verification that the transcript has been received by that institution.
Request for unofficial transcripts will be stamped "Issued Directly to Student."
Transcripts will not be issued to any individual who has not met all financial obligations to the Saint Vincent School of Nursing or is in default of state and/or federal loan programs.
Every effort is made to ensure the confidentiality of your transcript.
Graduates requesting a license in another state should follow the same procedure for requesting transcripts.
