Objectives:
At the conclusion of this activity, the participant should be able to:
-Discuss the development of the chronic care model and approach to caring for patients that have multiple medical conditions.
-Recognize the importance of engaging patients in their own care and strategies that may be successful in this approach.
-Apply clinical advances and new support processes for the long-term management of common chronic conditions.
